Today marks the launch of a new Care Quality Concern Form for professionals. Developed by the City of Wolverhampton Council’s Adult Social Care Commissioning Team this new form provides a clear and consistent way to report non-safeguarding care concerns and is available via the EMARF system.

What’s it for?

The form is designed to:

• Support care providers in self-reporting incidents, and
• Enable social care professionals to report concerns they observe in care settings.

This is not a replacement for safeguarding referrals. Instead, it complements the existing Safeguarding referral route, helping us to better capture and respond to quality-related issues.

Where will I find it?

The Care Quality Concern Form will be found alongside the Safeguarding Referral Form in the EMARF system.
